Please Complete Your Student's Impact Aid Survey Form
We have an urgent request that will directly impact the funding and support Oak Harbor Public Schools receives to benefit all our students. Each year, our district relies on families completing the annual Impact Aid Survey Form to secure critical funding for programs such as special education and other essential services. The survey must be submitted no later than Friday, October 16th.
It’s quick, confidential and essential that every family completes this form—even if you are not in the military or employed on federal property. No student information is shared and only numbers are used for reporting purposes.
This video will show you how to complete the form from your email and web browser. To view these instructions in another language, please click the CC button in the bottom-right corner to turn on closed captions, Select Settings, then Choose Auto-translate and select the language you want to translate to from the list of over 100 options.
App Instructions: To complete the form via the ParentSquare app, you will follow the same instructions in the video, but you will also need to select "Forms" at the top of the post message (see screenshot below).
